Use this form for less minor injuries (ie ones that probably will not require doctor's attention but still needed an icepack). Completed forms can be turned in at the Concession Stand.

This form needs to be filled out for more serious accidents, especially for insurance purposes. This form will be mailed to LL International and is used when an insurance claim needs to be submitted. Completed forms must be turned in at the Concession Stand within 72 hours of the accident. We have only 30 days to report incidents.
CLL Head Lice Policy
If a player is known to have head lice, the player must provide their own helmet for play, or not participate in the games until the head lice has been treated, nits are removed, and the child has been cleared to return to school. We do not want this to be any more of a hardship on families than it already is. If a family cannot provide its own helmet, please ask them to contact the equipment manager and we will find one for them to use.
